Aboriginal Health Promotion and Chronic Care (AHPACC)
The AHPACC program aims to improve health outcomes for Aboriginal Victorians through the prevention and better management of chronic diseases.

Victoria has nine AHPACC programs. The program for Melbourne’s eastern region is based on a primary partnership between EACH and Mullum Mullum Indigenous Gathering Place (MMIGP). It has links to other Aboriginal and mainstream health services and organisations.
The AHPACC program facilitates Aboriginal community access to appropriate health services and supports the Indigenous community identify, develop and participate in community orientated health promotion activity.
The AHPACC staff provide support to Indigenous people with, or at risk of, chronic disease and assist them in accessing culturally appropriate health care services such as:
- Doctors
- Detitians
- Physiotherapists
- Dentists
- Asthma Educators
- Community Health
- Podiatrists
- Social Workers
- Diabetes Educators
- Self help groups
The program is based between Maroondah Social and Community Health Centre and MMIGP.
