What is CREST?
CREST is a home-based outreach support service for adults who have a diagnosed mental illness living in the cities of Maroondah, Knox and Shire of Yarra Ranges.
CREST supports people to live independently in their community.
CREST also assists people to access and maintain secure, long-term, affordable housing.
What support does CREST provide?
The type and level of individual support can include:
- Assistance in developing independent living and coping skills such as managing a budget, running a household, using public transport and planning to meet day-to-day challenges;
- Linking people into local community services such as social groups, day programs, neighbourhood houses and gyms;
- Referral to specialist services such as therapists, case managers, doctors, employment, vocational, and rehabilitation services, and government departments and agencies;
- Assistance in accessing private, public and community housing; and
- Information, advice and advocacy.
Who is eligible?
CREST supports people who:
- Have been diagnosed with a mental illness;
- Are between 18 and 64 years of age;
- Want to live independently in the community;
- Need support due to their mental illness; and
- Live in or have links within the cities of Maroondah, Knox and the Shire of Yarra Ranges.
Who can refer?
People can refer themselves, or be referred by family, friends, health care providers and other service providers, but always in consultation with the applicant.
Referrals should be made to the Referral and Information Worker at the Ringwood office on 9879 2557.
