Hospital Admissions Risk Program (HARP)

HARP provides short term care coordination for people with complex needs.

What is HARP Psychosocial Care Coordination?

Psychosocial Care Coordination is provided to people at risk of frequent emergency department hospital presentations and who have complex psychosocial needs.

The aim of the program is to assist people to receive effective, timely and appropriate care in the community. Where possible, we work with people to reduce the frequency of presentations to hospital, maintaining better quality of life in the community.

The HARP Psychosocial Care Coordination staff work in conjunction with other HARP services such as allied health, nursing, chronic disease management and aged care.

What can HARP Psychosocial Care Coordination provide?

Care coordination may include:

  • comprehensive assessment
  • development of an agreed care plan with the client
  • consultation with other treatment providers involved in the client's care to promote a coordinated treatment plan and enhance communication between services
  • provision of short-term support and assistance wtih immediate needs
  • referrals to other specialist services
  • advocacy for the client
  • assistance to develop better self management techniques and linkages to community services
  • capacity to meet with clients in their home or in the community.

Eligibility

Clients with frequent hospital presentations and two or more of the following:

  • mental health issues such as depression or anxiety
  • drug and alcohol related concerns
  • problems with homelessness or risk of homelessness
  • social isolation or interpersonal issues
  • acquired brain injury (ABI)
  • difficulty with self management
  • difficulty linking to community

Referrals

Referrals to HARP Psychosocial Care Coordination are accepted from Emergency Department Care Coordinators, Hospital Social Workers, Health Services, Community Services and General Practitioners.
