The Heading Home program supports the successful transition of selected residents, from Secure Extended Care Units and the four Community Care Units in the Northern and Eastern Regions of Melbourne, into the community through providing a dedicated, intensive, sustained and integrated support response.

The three phases of the program are:

0. Preparation of the person for transition to the community
0. Provision of high level support in the community
0. Transition to ongoing support response in the community

Heading Home is a collaborative initiative between the Psychiatric Rehabilitation and Support Services and Clinical Mental Health Services.
