Reachout

Reachout provides intensive assertive outreach for people who are homeless or at the risk of homelessness and who have complex mental health issues.

The aim of the program is to assist clients to identify the factors that have contributed to their homelessness, and assist them in developing pathways for a sustainable recovery into independent living. Reachout covers Maroondah, Knox and the Shire of Yarra Ranges, working with people aged 16-65 years of age.

Reachout provides support by:

  • Providing intensive case management using individual recovery plan
  • Accessing services to enhance recovery
  • Assisting with forensic, drug and alcohol, trauma recovery, victims of crime, financial concerns and housing (either Office Housing applications or assistance with private rental applications)
  • Linking with a wide range of other services that are provided including:
    • clinical/general health care
    • social, and recreational activities
    • family reconciliation
    • educational and employment opportunities
    • negotiations with Centrelink
    • financial, problem gambling and drug and alcohol or other counselling

Reachout is a free community service funded by the Victorian Government Department of Human Services.
