Home Care Packages

The ​Home Care Package program is a Commonwealth Government funded program. The Home Care Package program provides individually planned and coordinated supports and services for older people to remain living life independently, the way you would like. 

The EACH Home Care Package program encourages people to actively participate in planning their services.

The EACH Home Care Package program is offered in:

  • NSW (Regions: Central Coast, Newcastle, Port Macquarie, South West Sydney)
  • QLD (LGAs of Brisbane, Ipswich)
  • TAS (LGAs of: Devonport, Hobart)
  • VIC (LGAs of: Banyule, Boroondara, Casey, Darebin, Knox, Manningham, Maroondah, Monash, Nillumbik Whitehorse, Whittlesea, ,Yarra, Yarra Ranges)

This program offers:

Services which can be organised as part of a package of care can include:

  • Advocacy
  • Basic garden and home maintenance
  • Basic nursing care including medication management
  • Home cleaning
  • Laundry tasks
  • Assistance with personal care
  • Meal support and preparation
  • Transport to appointments
  • Exploring respite options
  • Social support  

Who can access this program?

This program is available to older adults who need support and case management so they can remain living at home.

What does it cost?

The cost of this program is free for Older Adults with a full aged pension. For Older Adults with a part pension or other circumstances, a medicare assessed client contribution may apply. Please see attached fee schedule and call us to discuss.

When can this program be accessed?

Care Managers are available Monday to Friday 9:00am to 5:00pm with after hours support available. Support services are available at the times you need them.
