SHADES

SHADES aims to reduce the risk of people with a mental illness experiencing recurring homelessness.

SHADES:

  • Supports people in the community following discharge from the Inpatient Ward, Maroondah Hospital and links them in with the relevant community services and resources to prevent relapse and re-admission.
  • Ensures a smooth transition for people from short term intensive support towards longer term support matched with stable and appropriate housing.

The program operates on a psychosocial case management or care coordination model.

SHADES workers:

  • Assist in the identification and referral of people at risk of becoming homeless who are within the Inpatient Unit, Maroondah Hospital
  • Develop a plan for accommodation for the person upon their discharge
  • Provide intensive support to the person following discharge for a period of approximately 3 months.

Eligibility Criteria

Adults aged 18 to 64 who are patients of Maroondah Hospital, Mental Health Wards and homeless or at the risk of homelessness at discharge from hospital.

Referrals

Referrals are only acvcepted from the Psychiatric Inpatient Unit, Eastern Health Social Work Department.
