Sixty-five Plus
Sixty-Five Plus (65+) offers home-based outreach support and group programs for people 65 years of age and older who have mental health issues or a psychiatric disability.

The programs provided increase people’s skills and confidence to access services in the community.
The suppor provided is appropriate to each individual's needs and can include:
- information, advice and advocacy;
- referral to, and liaison with specialist services such as therapists, case managers, doctors and government agencies;
- links with local community services such as social groups, day programs, and neighbourhood houses;
- assistance in developing independent living and coping skills, for example managing a budget, using public transport, and planning to meet day-to-day challenges
- meeting day-to-day challenges; and

emotional support and practical assistance.
Who can apply?
People 65 years of age and older who have been diagnosed with a mental illness and:
- need support due to their psychiatric disability
- live in the cities of Knox, Maroondah or adjoining areas of the Shire of Yarra Ranges
- are willing to work at developing their independent living skills

want to make their lives better by developing their social networks.
Who can refer?
Self, family, friends, clinicians, agencies and organisations in consulttion with the individual.
