Our diabetes service supports people with self-management, medical management and living well with diabetes . Our services are located in East Ringwood, Wantirna and Ferntree Gully.

Our credentialed diabetes educators are registered nurses who use a holistic approach to health care.

Services Include:

Individual appointments

Sessions assist clients with diabetes medication management, insulin initiation, stabilisation and support, insulin pump management, blood glucose monitoring, management of other diabetes related health conditions and assisting people to live well with diabetes.

Diabetes Wellness Program

Education group for people living with type 2 diabetes. Also available for Chinese groups.  See flyer for Diabetes Wellness Program.

IDEAS clinic - Integrated Diabetes Education & Assessment Service for Type 2 Diabetes - staffed by diabetes educators & specialist diabetes doctors from Eastern Health. GP referral required. See IDEAS flyer.

DAFNE - (Dose Adjustment of insulin For Normal Eating) - A 5-day course for people over 17 years with Type 1 Diabetes. Groups run in May and September and require individual appointments prior to course. See DAFNE flyer.

This program offers:

  • Support and guidance for clients with Type 1, Type 2 and pre- diabetes in self management and wellbeing.
  • ​Individual appointments.
  • Comprehensive diabetes assessments and group education.

Who can access this program?

This program is open at all who hold a Health Care card/Pension card; otherwise standard consultation fees are income dependent.

Exclusions include gestational diabetes and children under 17 years.

What does it cost?

This program costs $10.00 for Health Care/Pension Card holders and people on low incomes. This program costs $15.00 for people on medium incomes.

When can this program be accessed?

This program can be accessed Monday to Friday, 9:00am to 4:00pm
