Assistance for people with intellectual, physical, dual disabilities as well as acquired brain injury and complex care needs to develop or maintain skills to live independently in their own home or in the community.

This program offers:

  • Individualised support that enables people with a disability to purchase a range of supports and services to meet their needs. 
  • An individual support plan outlining goals and aspirations is developed in partnership with each individual and their carer/family. 
  • Information and support to access a range of community services is also provided.​

Who can access this program?

This program is available to people with intellectual, physical, acquired brain injury; dual disability or people with complex care needs. People who have funding under Individual Support Packages (ISP), Transport Accident Commission (TAC) packages, National Disability Insurance Scheme (NDIS) packages or for those who are able to self-fund.

What does it cost?

Schedule of fees available on request for Department of Human Services’ funded clients; self-funded clients on needs basis.

Day Service:

A day service providing group activities for people with an intellectual disability. Group activities meet the individual's goals and match their skills and interests.

Where:This program is based in the Eastern Metropolitan region of Melbourne, operating out of various community hubs in the cities of Whitehorse, Maroondah, Knox and Yarra Ranges.
When:Groups run Monday to Friday - 9:00am to 3:00pm during school terms.
Who:This program is available to people aged 18 years or older with an intellectual disability or multiple disabilities.

Group Options:

Support for people with an acquired brain injury, acquired disability or neurological condition to access community activities and options.

Group activities include:

  • woodwork
  • social dinner
  • social group
  • lawn bowls
  • singing
  • art
  • gardening
  • cooking
  • craft group
  • desktop publishing and digital media
  • swimming
  • ten pin bowling
Where:Groups are based in various venues through the Melbourne Eastern Metropolitan region in the cities of Whitehorse, Maroondah, Knox, Yarra Ranges.
When:Groups are run Monday to Friday (9:00am to 5:00pm). Night Groups Monday (6:00pm - 9:00pm) and Thursday (5:00pm - 8:30pm). Groups can run for 2 - 3 hour sessions. The groups operate 48/52 weeks of the year.
Who:This program is available to people with an acquired brain injury, acquired disability or neurological condition who are in receipt of funding from either the Department of Human Services or Transport Accident Commission.
