Our Support Coordination service connects NDIS participants with informal, mainstream and specialist services.
We work with you to build independence and improve quality of life and support you overcome any barriers that you may encounter. A trained Support Coordinator will work with you to ensure you have access to the required supports to achieve the goals outlined in your NDIS plan.
The Support Coordination team includes social workers, case managers, health professionals and people with extensive disability services training, offer co-ordination services that meet the needs and goals of NDIS customers requiring childhood services through to youth and adults who may have a wide range of psychiatric, cognitive, physical, sensory, acquired, neurological and/or psychosocial disability and complexity in their lives.
We offer NDIS Support Coordination support across the North Eastern, Eastern and South Eastern regions of Melbourne.
Specialist Support Coordination
Specialist Support Coordination is high-level support coordination designed for those experiencing complex support needs.
Specialist Support Coordination will assist you with the following:
- Connection with supports to meet your needs;
- Ensure your NDIS services are delivered in a consistent way;
- Address any barriers in your current situation and environment;
- Assist you to build greater capacity and resilience.
You can expect your EACH Specialist Support Coordinator to provide high quality support coordination with attention to detail. We do this by working closely with NDIS and mainstream service providers in order to achieve the best possible outcomes.
Psychosocial Recovery Coach (Southern Melbourne)
EACH is registered to provide Psychosocial Recovery Coach for NDIS participants. We are focused on providing quality services in a person-centred way that promotes everyone’s wellbeing.
A Recovery Coach is an NDIS funded worker that has mental health knowledge. A recovery coach can spend time with you, and people important to you, to get to know you and understand your needs. They can assist you to link with services and supports that include mental health services.
We work with you to build independence, reduce barriers and improve your quality of life. Our Recovery Coach can meet with you in person or over the phone to assist you to understand, implement your NDIS plan to achieve your goals.
