Our Diabetes Service supports people with self-management, medical management and living well with diabetes.
The Diabetes Service is located in East Ringwood and Ferntree Gully.
Our credentialed Diabetes Educators are Registered Nurses and use a customer-centric, holistic approach to health care.
These sessions assist clients with Type 1 and Type 2 diabetes medication management, insulin initiation, stabilisation and support, insulin pump management, blood glucose monitoring, management of other diabetes-related health conditions and assisting people to live well with diabetes. Diabetes Wellness Program:
Integrated Diabetes Education & Assessment Service for Type 2 Diabetes - staffed by diabetes educators & specialist diabetes doctors from Eastern Health. GP referral essential- to be sent electronically to Eastern Health.

DAFNE (Dose Adjustment of insulin For Normal Eating) is a 5-day course for people over 17 years with Type 1 Diabetes. Groups run in May and September and require individual appointments prior to course. Please contact our Service Access team on 1300 00 3224.
At EACH, we are closely monitoring this evolving situation and are following all advice and directives from health departments and agencies. Who can access this program?
This service is open to all who hold a Health Care Card/Pension Card; otherwise, standard consultation fees are income dependent.
