​EACH's NDIS Support Co-ordination is a service which involves connecting our clients with informal, mainstream and specialist services and supports our clients to build their independence and quality of life, whilst working with our clients to help them overcome any barriers they may encounter.

EACH’s Support Co-ordinators work together with our clients to access their required supports so that they can achieve the goals outlined in their plan.

To start with, the EACH Support Co-ordinator will work with clients to develop their skills and confidence to take control of their own NDIS plan.

The EACH NDIS Support Co-ordination team that consists of social workers, case managers, allied health professionals and people with extensive disability services training, offer co-ordination services that meet the needs and goals of NDIS customers requiring Early Childhood Intervention Services through to youth and adults who may have a wide range of psychiatric, cognitive, physical, sensory, acquired, neurological and/or psychosocial disability and complexity in their lives.

This program offers:

  • ​Services to connect our clients with informal, mainstream and/or specialist services.
  • Assistance to build our client's confidence to take control of their own NDIS plan.
  • Coordination services for customers that require Early Childhood Intervention Services.
  • Coordination services for customers; youth and adult, that may have a wide range of complex disabilities.

This program supports clients that are eligible for NDIS.
