  • $55.98 per hour + 11.5% Super + Generous Salary Packaging benefits of up to $18,550

The Right Care Better Health program supports people with complex and chronic health conditions by coordinating care that’s tailored to their needs. Working alongside GPs and other providers, we help clients stay well by simplifying healthcare and focusing on what matters most to them.

Your role, your impact

As a Registered Nurse Care Coordinator, you will play a vital role in improving the health and wellbeing of patients living with chronic and complex health conditions across South/East Melbourne. Using your nursing expertise, you will provide coordinated, individualised care focused on empowering patients to actively manage their health and access appropriate services.

Key responsibilities:

As a Registered Nurse, you’ll play a vital role in delivering high-quality, person-centred care. You will:

  • Perform comprehensive health assessments and support clients to understand and manage their conditions.
  • Identify and prioritise health-related issues.
  • Support clients to overcome barriers to accessing care and staying engaged with services.
  • Collaborate with clients to set goals and create care plans.
  • Coordinate referrals to internal and external providers, including through social prescribing.
  • Engage and collaborate with GPs and local health services.
  • Maintain accurate clinical documentation and participate in quality improvement initiatives.
  • Represent the program at internal and external forums as required.

Who we’re looking for

You’re a passionate, skilled Registered Nurse with a strong understanding of chronic disease management and the community health sector. You thrive in a collaborative environment, enjoy problem-solving, and build positive relationships with patients and partners alike.

You’ll bring:

  • Bachelor of Applied Science (Nursing) or equivalent
  • Division 1 Registered Nurse with AHPRA registration
  • RN Grade 2 Year 4 minimum experience
  • Clinical experience in chronic disease management and working with complex patients
  • Skills in care planning, health education, and patient-centred care
  • Strong communication and stakeholder engagement skills
  • Understanding of community, primary health, and the social model of health
  • Experience working with diverse populations including Aboriginal and Torres Strait Islander and CALD communities

Nice-to-have:

  • Experience working in general practice/community health and multidisciplinary teams
  • Familiarity with digital health tools and care coordination platforms
