Social Worker – Help Empower Lives Through Inclusive Health Support

  • Part-time, 2 days per week until July 2026
  • Based in Campbelltown on the lands of the Dharawal People
  • Hourly Rate: Up to $43.27 per hour + 11.5% Super + Generous Salary Packaging benefits of up to $18,550

About the Program
My Care Partners is a collaborative and person-centred program designed to improve health outcomes for people with chronic conditions by addressing the social, emotional, and practical barriers to good health. Our team integrates clinical and psychosocial support, partnering with patients, families, and community services to reduce hospital admissions and build health resilience.

Your role, your impact
As a Social Worker in the My Care Partners team, you will play a vital role in improving the health and wellbeing of individuals and communities by addressing the social determinants of health. You’ll provide short-term counselling, practical support, and advocacy to ensure patients have the tools, resources, and services needed to better manage chronic conditions and achieve their personal health goals.

You will work closely with Registered Nurse Care Coordinators and other healthcare professionals, delivering team-based, inclusive care tailored to individuals from diverse backgrounds, including Aboriginal and Torres Strait Islander Peoples, LGBTIQA+ communities, and people from culturally and linguistically diverse (CALD) communities.

What you’ll be doing

  • Conduct comprehensive psychosocial and risk assessments to support care planning.

  • Provide short-term counselling for issues including depression, anxiety, family dynamics, and grief.

  • Work collaboratively with nurse care coordinators to address psychosocial barriers to health.

  • Support patients to self-manage their conditions and connect with appropriate services.

  • Ensure inclusive, culturally safe care that reflects EACH’s values and community needs.

  • Participate in team case reviews and multidisciplinary meetings.

Who we’re looking for

  • Bachelor’s Degree in Social Work and eligibility for AASW membership.

  • At least 2 years’ experience in social work, with strong skills in mental health, short-term counselling, and risk assessment.

  • Advanced clinical judgement and an understanding of trauma-informed, inclusive care.

  • Confidence using digital health records and telehealth platforms.

  • A current driver’s licence is essential.
