About the Care Finder program
Here at Each we are looking for a Team Lead to join our new Care Finder program which aims to provide specialist and intensive assistance to help people in the Care Finder target population groups to understand and access aged care services and connect with other relevant supports in the community.

  • Part-time (0.8FTE) ongoing position (Tuesday to Friday)
  • Based in Lismore or Grafton with travel required. Hybrid working conditions.

What you’ll be doing
In this fast-paced role no two days will be the same! As Team Lead your duties will include:

  • You will provide senior consultation, line management and support to the Care Finder team who practise assertive outreach and explore and establish different ways to effectively engage with people in the Care Finder target populations.
  • Assist with the development, implementation, and evaluation of a robust service program.
  • In line with your commitment to customer care, you will Manage a 50% caseload of clients within the Lismore/Richmond Valley, Clarence Valley/Grafton areas.
  • Provide specialist and intensive assistance to help senior Australians who need intensive support, who could otherwise fall through the gaps, to understand and access aged care services and connect with other relevant supports in the community.
  • Build and establish relationships within the community to promote the Care Finder program and assist with reaching potential clients.

Qualifications

If you’re a born leader with a commitment to delivering a person-centred approach, and an understanding of the policy and funding environment of the aged care and community services sector, then you might be exactly who we’re looking for!

Experience, Knowledge, and Qualification/s required for the job

  • Qualification in Social Work, Human Services, Aged Care, Community Services or Health and/or relevant experience, and enjoy assisting older adults within our community.
  • Demonstrated experience in leading and managing a team.
  • Experience working with older adults within the community with an understanding of the supports and services that are available in the local community.
  • Exceptional interpersonal communication, negotiation, and problem-solving skills.
  • Ability to work remotely from a team when required.
  • Current NSW driver’s licence and vehicle.
  • Computer literacy and the ability to effectively manage a client data base.
