EACH Co-ordin8 is a service for people managing chronic health concerns.
Our qualified nurse will work with you to help you understand your condition and to coordinate the health and educational services that meet your needs.
Confidential, no-cost services are provided in Ipswich.
This program offers:
- Work with patients and their families to further their knowledge and learn to self-manage their condition.
- Liaise with medical practitioners, allied health professionals and other community providers to coordinate services and enhance communication within the multidisciplinary team.
- Support implementation of care plans, attendance at appointments and arrange access to these appointments when required.
