How we help
The Pathways to good health service offers health screening, assessment, planning and referral support for children and young people entering or re-entering out-of-home care.
We offer:
- Health checks and planning for children and young people entering or re-entering kinship, foster or residential care
- Support from health navigators who work alongside children, carers and support team create tailored care plans
- Referrals to our specialist assessment clinic, including paediatricians, speech pathologists, occupational therapists and child psychologists.
