  • Location: Based at our Ferntree Gully office with outreach within the community + Hybrid role 
  • Position: Part-time/Full-time negotiable (0.6 - 1FTE) ongoing position. 
  • Salary: $56.65ph + super + up to $18,550 in generous Salary Packaging benefits

Are you passionate about supporting older adults to live active, connected, and independent lives? Join Each as our Nurse Coordinator for the Victorian Seniors Lifestyle Program and play a key role in improving wellbeing among older Victorians.

About the Role
As the Nurse Coordinator, you’ll be the primary point of contact for participants, working collaboratively with families and multidisciplinary teams to develop and implement tailored action plans. You’ll coordinate person-centred supports across physical activity, nutrition, medication management, social connection, and health coaching, empowering participants to achieve their goals and navigate health and social care systems with confidence.

Key Responsibilities

  • Engage participants and conduct goal-setting appointments using a strength-based approach
  • Undertake clinical assessments (Timed Up and Go, Hand Grip Strength, Malnutrition Screening Tool, Patient Activation Measure)
  • Develop, implement, and review individualised goal plans
  • Provide ongoing motivation, health coaching, and regular check-ins
  • Coordinate core and supporting services across multiple providers
  • Facilitate communication between participants, carers, GPs, allied health, and community organisations
  • Identify and address barriers to care, including access, transport, and health literacy
  • Support referrals, transition planning, and seamless service navigation
  • Contribute to clinical governance, quality improvement, and program reporting

About You

  • Clinical experience in home/community, sub-acute, or aged care settings
  • Advanced assessment and clinical care planning skills
  • Strong interpersonal and communication skills
  • Experience in person-centred, inclusive service delivery
  • Ability to work independently and as part of a team
  • Proficient with MS Office and client information systems (e.g., TrakCare, Carelink+)

Qualifications

  • Tertiary qualification in Nursing (Bachelor or equivalent, unrestricted)
